Obtaining a Material Safety Data Sheet
A manufacturer or distributor of a chemical product must send material safety data sheets to the purchaser (CDC) with the initial shipment of a product/hazardous chemical and with each update of the material safety data sheet. Material safety data sheets will either accompany or precede the shipment. However, the manufacturer or distributor is not required to send material safety data sheets with subsequent orders of the same product to CDC.
To obtain a material safety data sheet on a product/hazardous chemical:
- Request the material safety data sheets on the Purchase Order specifying that it be sent directly to your mailing address (see Appendix A).
- Write or call the manufacturer or distributor to request the material safety data sheet (see Appendix A), or
- Request the material safety data sheet from the CDC Information Center by submitting CDC Form 0.1038, (see Appendix A), or calling them at 639-3396.
